  1. Simply complete the form on the ‘Advertise with Us’ page
  2. Ad Format Specifications: 300px wide x 100px high - Jpeg, GIF or PNG
  3. No flash and no animated gifs, please
  4. Submit ad format specifications via email to med...@noblivity.com as an attachment
  5. The website URL provided on the form is where the ad will point
  6. Upon approval, we will email you a payment form to fax back to our office
  7. There are no Set-up fees, however, changes to your ad once the ad is set up will incur a $5.00 service fee
